Assessment


The Assessment is a non-invasive, information-gathering short meeting with the person requiring a long term, key medical professionals and family members (where applicable). The Assessment is an essential information-gathering tool that enables us to narrow down and target the long-term care need. It takes into consideration:


• Potential Cognitive / Dementia Issues

  - Short and long term memory issues etc.


• Behavioral Issues

   - Aggressive, combative, etc.


• Physical Issues / Limitations

   - Ambulation, incontinence, paralysis, etc.


• Personality and Preferences

   - Community, care home or in-home care


• Financial information – Private Pay or ALTCS (Medicaid)

   - If private pay; cost expectations per month


• Area of Town Preferred


• Any Other Pertinent Information


Although an in-person assessment is always the preference, a information gathering phone assessment may suffice in long distance situations
